Virtually every technology developer and thoughtful implementer of IV workflow management systems has been inspired by Flynn and Barker’s “Observational study of accuracy in compounding i.v.admixtures at five hospitals,” published in 1997, in the American Journal of Health-System Pharmacists.
The study’s data revealed a mean error rate of 9%, meaning almost 1 in 10 IV products was prepared incorrectly prior to dispensing.
